<description><![CDATA[Oh! Thank you very much. I was under the impression that Na channel blockers would entirely halt the Na influx, will eventually stop the Na Gate and cease the QRS complex and continuous influx of Na was necessary to elicit a prolonged QRS complex. It never occurred to me that slowing the Na influx would retain the Na channel gate open and thereby prolong the QRS complex on the EKG. Since we have limited time to answer the qustion, a primary rationalization (i.e. Na channel Blockers) will deviate considerably from the right choice.<br/><br/>I am afraid I could not copy and paste the question as it violates rights. Else I Could point or emphasise my ambiguity by quotes, therefore I did mention the question ID No.Perhaps you could review the question assuming you are a concerned staff of the usmle. No consent needed for patients under 18 for ER situations, STD treatment, medical care during pregnancy and managment of addiction. So in this case, she gives the consent. Notice I did not mention abortion since this issue varies from state to state so it cannot be tested on the exam. Hope it helps.]]></description>

<description><![CDATA[Continuous depolarization will not prolong the QRS complex. Remember that cardiac AP is different from Skeletal muscle AP. Cardiac muscle is not tetanizable like skeletal muscle. Cardiac myocytes beat in synchrony meaning if 1 myocyte contracts all the other myocytes contract, where as skeletal muscle cells if one depolarizes the other cells will not depolarize at the same time. Cardiac myocytes are special in that they can beat on their own without nerves innervation, where as skeletal muscle needs nervous input in order to produce AP. If you cut the nerve going to a skeletal muscle it atrophies and cannot longer produce AP(this is not true for cardiac muscle). Symp. and Parasym. input only modulate cardiac myocytes; meaning they either increase or decrease the already pre-set rhythm by myocytes. If you want to prolonge the ventricular QRS then you have to slow down the Na influx and that is what Propafenone does. Since it is an amide it is metabolized quickly by the liver and therefore has a short t1/2 life. Also, remember that most arrythmias start in the RRP (phase 3)not during QRS, and during phase 3 you have both Na and K ions involved. I am not sure what your question is but I hope that this general statement helps.]]></description>

<description><![CDATA[Hello Aksh,<br/>thanks for your replay. I have searched different books on this subject and they all give different meaning. I have looked into Anatomy books, review books by Kaplan, Falcon and BRS and they all disagree on the subject. So here is what I have figured out on my own, correct me if I am wrong on any of this. Clawing involves the Lumbrical muscles not the interossias muscles. The function of the lumbricals is to extend the MCP joint, so these are so called "bye-bye" muscles. It makes sense to me that if the lumbricals of the 2nd and 3rd digits are "clawed"-meaning they are flexed and therefore cannot extend that the median nerve is involved, and if the 4th and 5th lumbricals are "clawed" that ulnar nerve is involved. But here is another confusing thing. According to my findings, both the ulnar nerve and median nerve clawings occure at rest, as if the patient just walked into your office and you as a doc observe it. Some textbooks say it occures only if you ask the patient to flex or extend the digits. <br/>Maybe the terms "Ape hand" "Clawing" "Pops blessing" "Preacher's hand" are confusing to me since English is my 4th language and these terms have no meaning to me (without the proper context). Correct me if I am wrong on any of my findings. Thank you.]]></description>

<description><![CDATA[Can anybody explain the differences b/w these 2 injuries, and different types of "clawing" please?<br/>According to Kaplan, if all 4 lumbricals are involved that represents clawing of all 4 lumbricas aka Klumpke's palsy. But this is where it gets confusing. Kaplan says that if 2nd and 3rd digit is clawed that is median n. injury and if 4th and 5th digit it represents ulnar nerve injury. This makes sense to me since medial side (thumb) is innervated by the median nerve and lateral side by the ulnar nerve, but than 1st Aid has the nerve injuries opposite. Can anybody pls explain to me if I am right or wrong on this on? Thanks alot.]]></description>
